INFORMED CONSENT

TITLE: Perceptions related to modern technologies used for mental health and suicide prevention support

This is a study about perceptions related to modern technologies used for mental health and suicide prevention support. You have been invited to participate in this study, and you can choose to take part or not. If you do decide to participate, you will be asked to complete an online survey that will take about 10-20 minutes to finish.

The survey will ask you questions about modern technologies used for mental health and suicide prevention support. Some questions will ask you to rank something on a scale, some questions will ask for a yes or no answer and there will be some optional questions where you can write a response. You can stop participating in the survey at any time without penalty.

There are minimal risks associated with this study. You may become embarrassed or uncomfortable with some of the questions, but you can stop participating at any time without penalty. There are no treatments available in the event of an injury or discomfort. However, you will receive $100.00 for completing the survey.

The information you provide will be kept confidential to the extent allowed by law. Your name and email address will be collected for payment purposes using a separate collection tool to protect your privacy. You will not be identified in any reports or publications about the study.

Participating in this study is voluntary. Your decision whether or not to participate will not affect your current or future relations with Aptive. If you have any questions or concerns, you can contact the VXC research team at VXC@Aptiveresources.com.
